Childs U.S. Passport
In order to maintain security when applying for a U.S. Passport, all passport applicants are required to show up in person at their local passport acceptance agent. These agents are located at Post Offices and County Clerk’s Offices across the country. Search for your acceptance agent here.

Assemble ALL of the documents below and go to your local Acceptance Agent (post office)
Tell the Acceptance Agent you intend to use “HAND-CARRY SERVICES”
The Acceptance Agent will take your documents, sign off on them, stamp them, and seal them in an envelope
You then take the “security envelope” back from the acceptance agent and ship the application (details below):
Put the (Sealed Security Envelope + PhiladelphiaPassport.com Order Form + One Letter of
Authorization + Proof of Travel) inside a UPS/FedEx shipping envelope and UPS/FedEx it to the mailing address at the top of the order form.

Step 1: Gather and Prepare Documents for a Expedited Child U.S. Passport

a. Online DS-11 Application – Do not sign this application form until you are instructed to during Authorization.

Follow this link to fill out the DS -11

Important, When finished you will click the Link that says “create form”.
Once printed there should be Two(2) barcodes on the page labeled Page 1 of 2

b. PARENTAL CONSENT

Important – If the child is under the age of 16:

Both parents must sign the application for passport in presence of the acceptance agent or if only one parent can be present you must have a signed, notarized Parental Consent Letter from the non-present parent. If sending in a Parental Consent Letter, the non-present parent must submit a clear copy of their Drivers License.

Only one parent needs to appear if that parent can prove they have sole custody to apply for the child

c. Proof of Citizenship – You must provide ONE of the following.

Certified Birth Certificate issued by state, county or city
Certificate of Citizenship or FS-240, DS-1350
Certificate of Naturalization

d. PROOF OF IDENTIFICATION FROM BOTH PARENTS

Valid State or Federal ID.

e. Evidence of Immediate Travel – supply 2 copies of 1 of the following

Copy of ticket indicating travel to a foreign country.
Travel itinerary or eticket stating reserved or confirmed.
Business travelers may submit a Business Letter of Expedite, if the above is not available. Call for clarification.

f. Passport Photographs – taken by a professional.

Obtain 2 Passport photos. (Pharmacies do this. CVS, Walgreen’s etc.)

g. U.S. Government Fees

Make a check or money order payable to “U.S. Department of State” for $160 (this is the government processing fee for an Expedited Passport). Check should have a pre printed name and address. Temporary or starter checks are not acceptable.

Acceptance Agent Fees are $35

h. Authorization Letter- Allows us to act as your agent.

Print and complete 2 copies of the Expedited Passport Authorization Letter

Print our Order Form, complete and include with order.

Step 2: Acceptance Process
An Acceptance Agent will verify your identity and witness your signature

Passport Acceptance Agents are found in local Post Offices and Court Houses. (not every
Post Office or Court House is an Acceptance Agent) Most acceptance facilities are open 9:00am – 4:00pm. We urge you to call your local Acceptance Agent to verify their hours of operation. Click Here to search by zip code for the nearest agent. Please be sure to check the hours they  process passport applications.

Before appearing in front of an Acceptance Agent, please be sure you have all the required documents.

This is what you will need.

1. Completed DS- 11 application
2. Parental Consent Letter (If Applickable)
3. Proof of Citizenship
4. Proof of Identity
5. Evidence of Immediate Travel
6. Authorization Letter
7. Passport Photographs
8. The $140 check or money order to “U.S. Dept. of State”
