for Obtaining Extra Passport Pages

Step 1: Gather and Prepare Documents to Expedite Additional Pages on Your U.S. Passport
a. Signed DS-4085 Application
Follow this link to fill out the DS-4085
Important When finished you will click the link that says “create form”.
Once printed there should be Two(2) barcodes on the page labeled Page 1
b. Your most recent U.S. Passport
Your current undamaged U.S. Passport must have at least 12 months validity remaining, otherwise, you will need to renew your passport.
c. Evidence of Immediate Travel – supply 1 copy of 1 of the following
Copy of ticket indicating travel to a foreign country.
Travel itinerary or eticket stating reserved or confirmed.
Business travelers may submit a Business Letter of Expedite, if the above is not available. Call for clarification.
d. U.S. Government Fees
Make a check or money order payable to “U.S. Department of State” for $142.00 (this is the government processing fee for Extra Pages). Check should have a pre printed name and address. Temporary or starter checks are not acceptable.
e. Authorization Letter- Allows us to act as your agent.
Print and complete 1 copy of the Expedited Passport Authorization Letter
f. PhiladelphiaPassports.com,Inc. Expedited Order Form
Print our Order Form, complete and include with order.

Important ! DO NOT USE U.S. Postal Service Express Mail.
Please place the following documents in a FedEx or UPS Express envelope.
1. Signed DS-4085 Application
2. Your most recent passport

4. Authorization Letter
5. The $142 check or money order to “U.S. Dept. of State”
