RUSH Processing Instructions for a
Lost or Stolen U.S. Passport Replacement

In order to maintain security when applying for a U.S. Passport, all passport applicants are required to show up in person at their local passport acceptance agent. These agents are located at Post Offices and County Clerk's Offices across the country. Search for your acceptance agent.

Assemble ALL of the documents below for your lost or stolen passport and go to your local Passport Acceptance Agent (post office) Tell the Acceptance Agent you intend to use "HAND-CARRY SERVICES" The Acceptance Agent will take your documents, sign off on them, stamp them, and seal them in an envelope You then take the “security envelope” back from the acceptance agent and ship the application (details below):

Put the (Sealed Security Envelope + PassportGuys.Com Order Form + One Letter of Authorization + Proof of Travel) inside a UPS/FedEx shipping envelope and UPS/FedEx it to the mailing address at the top of the order form.

  Step 1


  Gather and Prepare Documents for a Expedited Replacement for a Lost or Stolen U.S. Passport
 

    a. Online DS-11 Application - Do not sign this application form until you are instructed to during Authorization.

      Fill out the DS-11

      • Online DS-11 Application (Follow instructions on Government page you will be sent to)
      • Important, When finished you will click the Link that says "create form". Once printed there should be Two (2) barcodes on the page labeled Page 1 of 2

    b. Application Form DS-64 - Statement regarding lost or stolen passport.

    c. Proof of Citizenship - You must provide ONE of the following:

      • Previous U.S. Passport more than 15 years old.
      • Certified Birth Certificate issued by state, county or city
      • Certificate of Citizenship or FS-240, DS-1350
      • Certificate of Naturalization

    d. Proof of Identity

      • Valid drivers license with photograph issued more than 6 months ago. If you do not have a drivers license or a drivers license that meets the requirements above then you must supply Personal Documents.

    e. Evidence of Immediate Travel - supply 2 copies of 1 of the following:

    f. Passport Photographs - taken by a professional.

      • Obtain 2 Passport photos. (Pharmacies now do this. CVS, Walgreen's etc.)

    g. U.S. Government Fees

      • Make a check or money order payable to "U.S. Department of State" for $170 (this is the government processing fee for a Lost or Stolen Passport). Temporary or starter checks are not acceptable.
      • Acceptance Agent Fees
      • Make the second check or money order in the amount of $25 payable to the Post Office or Court House which you visit.

    h. Authorization Letter - Allows us to act as your agent.
